HYROX knee pain is one of the most reported complaints. It often traces back to the running and lunging combination performed under fatigue. As your hip abductor muscles tire, they lose their ability to keep the femur tracking straight.
Your knee drifts inward with every stride and every lunge rep, increasing patellofemoral load with each repetition. Over a race, or over months of HYROX training, that repeated inward drift is what produces patellofemoral pain, patellar tendinopathy, and IT band irritation.

1. SkiErg (1,000m)
The primary risk is for your shoulders and upper back. Scapular fatigue tends to set in by 600 meters. Once it does, the stroke starts pulling from your shoulder joint instead of the back, precisely the reason for HYROX SkiErg shoulder pain.
2. Sled Push (50m)
Lower back, knees, and calves absorb most of the load here. Technique often degrades fast under a heavy sled. More importantly, a rounding back or collapsing knee angle increases the risk of sled push injury at this HYROX station.
